Client Intake & Training Agreement

I confirm the total amount due above is correct and agreed.

I understand the Trainer provides private one-on-one training and does not guarantee specific results; outcomes depend on consistency, environment, genetics, and handler compliance.

I agree to provide truthful disclosure of my dog's behavioral history, follow Trainer instructions and training plans, maintain control of my dog at all times, use only Trainer-approved equipment, and practice safe, humane, Trainer-approved techniques at all times.

I understand Moore Canine Co., LLC maintains a strict zero-tolerance policy for abuse, neglect, intimidation, or unsafe handling of any dog, and that any evidence or suspicion of abusive conduct will result in immediate termination of services with no refund and may be reported to the appropriate authorities.

I confirm my dog is in good health and up to date on vaccinations as required by law, and I will notify the Trainer of any illness, injury, or regression prior to sessions.

I understand payment is due in full prior to or at the first session unless otherwise agreed in writing, packages are non-transferable between dogs or handlers, and all sales are final.

I understand a minimum of 24 hours notice is required to cancel or reschedule, and that late cancellations and no-shows are charged in full.

I understand no e-collar may be used on my dog at any time while working with Moore Canine Co., LLC unless explicitly instructed and approved by the Trainer; unauthorized use constitutes a material breach and may result in immediate termination without refund; and I must complete all prerequisite homework before any e-collar introduction.

I agree to complete all assigned homework between sessions and submit requested videos demonstrating homework completion; failure to submit required videos may result in immediate cancellation of services without refund.

I understand dog training involves inherent risks. I assume all risk and release Moore Canine Co., LLC from liability for injury, damage, or loss caused by my dog, and I am responsible for all damages or injuries caused by my dog.

I understand the Trainer may terminate services immediately for unsafe behavior, failure to follow instructions, unauthorized e-collar use, failure to complete homework/videos, or non-payment/repeated cancellations, without refund, and that this Packet is governed by the laws of the state in which services are provided.
