Board & Train Intake & Training Agreement

I authorize Moore Canine Co., LLC to seek veterinary treatment as necessary. I understand all veterinary expenses are my responsibility.

I confirm my dog is free of contagious illness at the time of intake.

I acknowledge that failure to disclose behavioral issues or bite history voids all liability protections and may result in immediate termination of services.

I have provided complete and truthful information regarding my dog.

I agree to follow all training, handling, and equipment instructions exactly.

I understand results depend heavily on my consistency and follow-through.

I agree to attend required transfer or education sessions when applicable.

I understand failure to comply may result in termination without refund.

I understand Moore Canine Co., LLC maintains a zero-tolerance policy for abuse, threats, harassment, or intimidation toward staff or animals, and that violations result in immediate termination without refund.

I agree that no electronic or electric training device of any kind (including e-collars, remote collars, electric containment/fence systems, GPS collars, vibration collars, and tone-based collars) may be used during the duration of training unless expressly authorized, introduced, and instructed by Moore Canine Co., LLC. Any unauthorized use constitutes a material breach resulting in immediate termination without refund.

I confirm all prior and current use of any electronic training or containment device has been fully and accurately disclosed, including type, duration, frequency, and any known adverse reactions. I release Moore Canine Co., LLC from liability for complications arising from undisclosed, inaccurate, or improper prior device use.

I agree to complete assigned homework.

I agree to submit requested videos or follow-up proof when required.

I understand any training equipment required by the Trainer must be paid for by me, typical equipment costs range from $40–$100 (not including e-collars), all purchased equipment is non-refundable, refusal to approve or pay for required equipment constitutes noncompliance and may result in modification, suspension, or termination of services without refund, and that once equipment is issued, responsibility transfers to me with lost/damaged/destroyed equipment billed at full replacement cost.

I understand dates may be transferred only until this agreement is signed.

I understand one (1) reschedule may be granted after signing.

I understand two (2) cancellations result in forfeiture of future services with Moore Canine Co.

I understand that after seven (7) consecutive days of zero communication, my dog will be considered abandoned and ownership will transfer to Moore Canine Co., LLC.

I understand there are no guarantees of training success and that dogs are animals, not machines.

I voluntarily assume all risks associated with dog training and boarding, including injury, illness, escape, or behavioral incidents.

I grant Moore Canine Co., LLC the irrevocable right and permission to photograph, record, and use my dog's name, likeness, image, voice, and training footage in any media format for lawful business purposes, and I understand all media content created during training is the sole property of Moore Canine Co., LLC with no compensation provided.

I understand Moore Canine Co., LLC may terminate services at any time due to safety concerns, dishonesty, noncompliance, or welfare considerations without refund, and that this Agreement is governed by the laws of the state in which Moore Canine Co., LLC operates.

I understand all payments are non-refundable once signed.
