Vacation Boarding Agreement & Liability Waiver

I certify that my dog is in good health and free from known contagious illness. I have disclosed all medical conditions, medications, allergies, behavioral concerns, bite history, aggression, fears, anxieties, and escape tendencies.

I certify my dog is current on all required vaccinations and records have been provided prior to boarding.

I understand boarding involves inherent risks including illness, injury, stress-related behaviors, escape attempts, self-inflicted injury, unforeseen medical emergencies, and death. I voluntarily assume all such risks.

I authorize Moore Canine Co., LLC to seek veterinary care if necessary. Reasonable efforts will be made to contact me first. I accept responsibility for all veterinary expenses incurred.

I release and hold harmless Moore Canine Co., LLC, its owners, employees, contractors, and representatives from claims arising from illness, injury, death, property damage, behavioral incidents, veterinary treatment, or events occurring during boarding, except in cases of gross negligence or intentional misconduct.

Moore Canine Co., LLC is not responsible for lost, damaged, destroyed, or soiled personal items left with the dog.

If the dog is not picked up and no communication is received within seven (7) days after the scheduled pickup date, Moore Canine Co., LLC reserves the right to pursue legal remedies, including rehoming or arranging alternative care.

Additional fees may apply for veterinary care, emergency transportation, extended stays, special care requirements, or property damage caused by the dog.
