I understand that this form is used to determine whether my dog and training goals are an appropriate fit for services offered.

I agree to provide complete and truthful information regarding my dog's medical and behavioral history.

I understand failure to disclose behavioral or bite history may result in refusal or termination of services.

I understand no training outcome or behavioral modification results can be guaranteed.

I understand training success depends heavily on owner participation and consistency.

I acknowledge Moore Canine Co., LLC reserves the right to refuse services if safety concerns exist.

I understand submission of this form does not guarantee acceptance or scheduling.

I understand that dogs are animals and may behave unpredictably. I agree to hold harmless and release Moore Canine Co., LLC, its owners, employees, contractors, and representatives from liability for injuries, damages, losses, or expenses resulting from information provided on this form or from my dog's behavior, except in cases of gross negligence or willful misconduct.

I understand and agree that Moore Canine Co., LLC may photograph, video record, and otherwise capture media of my dog, myself, members of my household, and my property during evaluations, training sessions, boarding, field trips, and any other services provided. I acknowledge and agree that all photographs, videos, recordings, and other media captured by Moore Canine Co., LLC are and shall remain the sole and exclusive property of Moore Canine Co., LLC. I grant Moore Canine Co., LLC the unrestricted, perpetual, irrevocable right to use, reproduce, edit, publish, distribute, display, and otherwise utilize such media for training, educational, marketing, advertising, social media, promotional, and any other lawful business purposes without compensation or further approval.
